In Sweden, therapy through the public system is close to free. Getting it, on your terms and in your language, is the hard part. The system decides who you see, how many times, and what kind of treatment you get. For many expats it works. For many it does not, which is why private practice in English exists at all. Here are the 2026 rules.

I will cover the public route through your vårdcentral and what it costs. The care guarantee, and what it does and does not promise. Private practice, and insurance. Then where I fit, and where I do not.

The front door is the vårdcentral

There is no separate referral for a psychologist in Sweden. You book at your vårdcentral, the health center you are listed with, and ask for help with psykisk ohälsa. Many centers have a psychologist or a counselor, a kurator, on staff. Others send you to a private clinic they contract with. Either way you pay the ordinary patient fee. For heavier problems the vårdcentral refers you on to specialist psychiatry. Access is by need. Milder problems get short, structured help, often a handful of sessions built on CBT. Severe ones get more.

What you pay: the fee and the cap

The patient fee is set by your region. A visit to a psychologist or kurator at the vårdcentral typically costs 100 to 350 kronor. Some regions charge less for video visits. All of it counts toward the högkostnadsskydd, the high-cost protection. In 2026 the cap is 1,450 kronor in every region for a twelve-month period. Once you reach it you get a frikort and pay nothing for the rest of the period. Under 18s pay nothing. This is the cheapest therapy in Europe, if the system gives you enough of it.

The care guarantee, and what it does not promise

The vårdgaranti sets three limits. Contact with your vårdcentral the same day. A medical assessment within three days. A specialist visit, and the start of treatment, within 90 days each. That is the outer limit, not the typical wait. It says nothing about who you see or in what language, and nothing about how many sessions you get. Waits and offers vary a great deal between regions. Psychologist time at vårdcentral level is the scarce resource. If your problem is moderate, the system may offer you a group course or a digital program rather than weekly sessions.

Private practice: what it costs

Outside the region, psychologists and licensed psychotherapists set their own fees. In 2026 a private session usually costs 800 to 1,800 kronor, with Stockholm and Gothenburg at the top. The high-cost cap does not apply. No referral is needed, you choose your therapist and your language, and nothing enters your journal. Many people have private health insurance through their employer that pays for a set number of sessions. Some home insurance policies cover sessions after a crisis or an accident. Check both before you assume you have nothing.

When you should stay in the system

If you need medication, your vårdcentral or specialist psychiatry is the route. If you need a diagnosis, for sjukskrivning, for Försäkringskassan or for anything official, you need a legitimerad clinician inside the system. If you are in crisis, the psychiatric emergency service is the right door. And if your problem fits what the vårdcentral offers, it is nearly free. Book the assessment, and decide what to do with what they offer you.

How I fit, and where I do not
